How a bill like this happens

  • The program was out of network, and nobody said so plainly
  • The plan “allowed” a fraction of the billed rate
  • Insurance stopped approving days, and the stay continued
  • Lab testing was billed separately, at very high rates

None of this is a secret to the program. It should not be a secret to you.

Questions

About cost

How much does rehab cost?

It depends on the level of care and your insurance. Billed rates commonly range from a few hundred dollars a day for outpatient programs to more than a thousand dollars a day for detox and residential care. What you pay is set by your deductible, coinsurance and out-of-pocket maximum.

Why did I get a bill after insurance paid?

Usually for one of three reasons: you had not met your deductible, your coinsurance applied to a large bill, or the program was out of network and billed you for the difference between its charge and what your plan allowed.

What is balance billing?

Balance billing is when an out-of-network provider bills you for the gap between what it charged and what your insurance allowed. That amount does not count toward your out-of-pocket maximum.
