YOUR HEALTH PLAN PPO
Sample card
Member ID
Identifies you. The first three letters on a Blue Cross Blue Shield card are a prefix that tells providers which Blue plan to bill.
Group number
Identifies your employer’s plan. Two people with the same insurer can have completely different benefits. This number is why.
Plan name and type
HMO, EPO, PPO or POS. It decides whether you can go out of network and whether you need a referral.
Copays
Flat fees for office visits. Treatment programs are usually billed under your deductible and coinsurance, not these copays.
RxBIN, PCN, RxGroup
Pharmacy routing numbers. They tell the pharmacy who pays for prescriptions.
Behavioral health phone number
Often a different company from your medical insurer. This is the number to call about treatment.
