What kind of plan do I have?
The letters on your card decide where you can go.
HMO
Lowest cost. You stay in the network and usually need a referral. Out of network is not covered except in an emergency.
EPO
No referral needed, but still no out-of-network coverage.
PPO
Most freedom. You can go out of network, and you pay more when you do.
POS
A mix. Referrals like an HMO, some out-of-network coverage like a PPO.
HDHP
A high deductible, often paired with a health savings account. You pay the full price until the deductible is met.
Medicaid
State coverage based on income. Covers addiction and mental health treatment, at programs that accept it.
Medicare
Covers inpatient, outpatient, and opioid treatment programs. Rules differ between Original Medicare and Medicare Advantage.
TRICARE
Military coverage. Covers detox, residential, PHP, IOP and medication treatment, usually with authorization.
